Member Forms

Please note that you must be logged in to your MySDRS account to access member forms online.

Click the "Log in using MySD" icon in the upper right-hand corner to link/access your MySDRS account. Once logged in, forms can be located in the MySDRS dropdown. Forms available for download through your MySDRS account include:

• Applications for SDRS Retirement Benefits (B-2F for Foundation members, B-2G for Generational members)
• Application for SDRS Refund Benefit (B-4)
• Personal Information Change Request (E-2) for mailing address, marital status, and name changes
• Beneficiary Designation Form (E-5)
• Transfer to Minor Form (E-5A)
• Authorization for Direct Deposit of Benefit Payments (B-6) for changes to bank account information for direct deposit of monthly benefit payments
• And more.

Member Statements/Tax Documents

Are you looking for your annual Personal Benefits Statement, Retiree Statement, or 1099-R tax document? To access these documents, you must first be logged into your MySDRS account. Click the "Log in using MySD" icon in the upper right-hand corner to link/access your MySDRS account. Once logged in, click on the "MySDRS" dropdown in the navigation bar at the top of the page (or the menu bar if using a mobile device). Then click "Documents."

 

Survivor Forms

Applications for SDRS Survivor Benefits are available through the SDRS office only. For assistance with survivor forms, please call the SDRS office at 605-773-3731.
